Sponsor form checklist

OK. You've walked the walk, had bags of fun and raised lots of money. But your hard work is not over just yet. We need you to round up your sponsorship money, complete the sponsor form and send it along with a cheque to the address below.

But wait! Before you proudly pack your sponsor form into an envelope, you need to ensure the following information is on your sponsor form:

• Your 'Walker Number' is written in the space provided on the sponsor form . This is very important. Your number was sent to you in the confirmation email when you registered but if you need a reminder then just call 029 2067 2053 or email sali.griffiths@tyhafan.org
• Enter your full name, address and email address in the space provided
• Gift Aid it! When you're collecting your sponsorship, ask each person to tick the Gift Aid box. By doing this we'll receive an extra 28 pence from every pound you raise.
• Return your completed sponsorship form with the cheque to:
